The minimum number of players a bookie or agent would need to set up with a Price Per Head service or a Pay Per Head company differs across the board.

Gambling911.com endorsed Real Bookies only requires five players to get started.  

Some PPH outfits claim there are no minimums though it goes without saying you should have at least a handful of players before entering into a business arrangement with a Price Per Head contracting service if you desire to be taken seriously.

Companies like RealBookies.com charge a fee of $10 and up per customer depending on the type of service required.  You will not be assessed additional fees and Real Bookies only charges for active players.
